Seventeen male patients aged between 20-44 years old, infected with human immunodeficiency virus (HIV), who developed cutaneous symptoms and biochemical signs of Porphyria Cutanea Tarda (PCT) between 2 months and 5 years after detecting HIV infection, are presented. Diagnosis of PCT was confirmed based upon increased urinary porphyrin excretion with pattern and plasma porphyrin index (PPI) typical of this porphyria. Inheritance nature of the disease was stablished on the basis of familial data and on diminished erythrocyte Uroporphyrinogen Decarboxylase (URO-D) activity. The seventeen cases of HIV-PCT here presented represent 30% of total cases reported in the world literature. Two patients were treated with the combined therapy: oral S-Adenosyl-L-Methionine (SAMe) and chloroquine, after 7 months of treatment the urinary excretion of porphyrins diminished to half initial values. Since in all of these patients porphyria developed after detection of virus infection, it is proposed that HIV or some associated factors could act as precipitating agents in the symptomatic expression of either acquired or familial PCT.
