Abstract:
The goal of this presentation is to provide guidelines for the performance of endoscopic and percutaneous procedures for the management of acute diseases of the pancreas and the hepatobiliary tract. As more experience is gained with these procedures, more effectives are the results obtained, with diminution of the morbidity and mortality as compared with the conventional surgical management. The presentation and subsequent management of 133 patients with acute biliopancreatic diseases treated with drainages performed by endoscopic route in 72 cases and by percutaneous route in 61 cases, are reviewed. In high-risk patients with acute diseases of the pancreas and the hepatobiliary tract, the invasive non-surgical management proved efficacy for both percutaneous and edoscopic approaches. Of the patients treated by endoscopic route, 49 were cholangitis and 29 biliary acute pancreatitis. Of 61 patients treated by percutaneous route 49 were cholangitis and 12 acute cholecystitis, and lithiasis was the most common cause of acute infection in 91 cases, and 10 were tumours, 26 benign stenosis and 1 case was a sclerosant cholangitis. Morbidity was 6,9% for the endoscopic route and 13% for the percutaneous route, with an overall morbidity rate of 9,7%. Mortality originated in the endoscopic route was 5,5% and for the percutaneous route it was 4,9%, with an overall mortality rate of 5,2%.
